Second-Line Chemotherapy for Platinum- and Taxane-Resistant Epithelial Ovarian Cancer: Pegylated Liposomal Doxorubicin (PLD), Irinotecan, and Combination Therapies at Lower Doses

چکیده
Epithelial ovarian cancer is sensitive to chemotherapy and approximately 75% of patients achieve complete clinical remission after the initial treatment. However, most develop a recurrence which results in death after a chronic course. Therefore, most patients are candidates for second-line chemotherapy, including approximately 20% among of these patients with platinumand taxane-resistant disease. Patients with platinum-and taxaneresistant disease have poor outcomes, and most would like to prolong their survival with relieved symptoms and the best possible quality of life (QOL). The effects of several drugs which are being used for these purposes are similar, but it is usually difficult to relieve recurrent disease with one drug. Monotherapy has been generally chosen for having the most favorable toxicity profile in patients with platinumand taxane-resistant disease. However, it is not clear whether this strategy is optimal with the various novel anticancer drugs and targeted agents under development. In this article, pegylated liposomal doxorubicin (PLD), which is used worldwide, and irinotecan, the predominant agent in Japan for the treatment of platinumand taxane-resistant ovarian cancer, are evaluated for their effects and toxicity in monotherapy and in combination at lower doses with other drugs.

Real-life experience using trabectedin plus pegylated liposomal doxorubicin combination to treat patients with relapsed ovarian cancer
The goal of recurrent ovarian cancer (ROC) treatment is no longer just palliation, but prolonging survival. This is usually through administering a new line of chemotherapy at each relapse. A novel treatment sequencing strategy to achieve this is through the intercalation of an effective non-platinum alternative, in between platinum-based therapies. Trabectedin in combination with pegylated lip...
متن کاملEffect of the Combination of Trabectedin and Pegylated Liposomal Doxorubicin in a BRCA2 Mutation Carrier with Recurrent Platinum-Sensitive Ovarian Cancer
INTRODUCTION The current standard of care for ovarian cancer is optimal cytoreduction with adjuvant chemotherapy based on a platinum/taxane combination. Although the response rate to this therapy is high, most patients ultimately relapse. Response to second-line therapy and prognosis are linked to the platinum-free interval (PFI); when both improve, the PFI increases. As a result, there is an i...
متن کاملIntroduction to managing patients with recurrent ovarian cancer
Ovarian cancer is the 5th most common cancer found in women in the UK. It is the leading cause of death from gynaecological cancer, and is the 4th most common cause of cancer death among UK women. Similar to the majority of other cancers, relative survival rates for ovarian cancer are improving, although 5-year mortality rates remain stubbornly low. The stage of the disease at diagnosis is the ...
متن کاملPegylated liposomal doxorubicin in the management of ovarian cancer
Among the pharmaceutical options available for treatment of ovarian cancer, much attention has been progressively focused on pegylated liposomal doxorubicin (PLD), whose unique formulation, which entraps conventional doxorubicin in a bilayer lipidic sphere surrounded by a polyethylene glycol layer, prolongs the persistence of the drug in the circulation and potentiates intratumor drug accumulat...
متن کاملPegylated liposomal doxorubicin: appraisal of its current role in the management of epithelial ovarian cancer
Pegylated liposomal doxorubicin (PLD) has become a major component in the routine management of epithelial ovarian cancer. The drug is frequently employed as a single agent in the platinum-resistant setting, and recently reported data reveal the superiority of the combination of PLD plus carboplatin, compared with the platinum drug plus paclitaxel, in delaying the time to disease progression in...
